Purpose of this form:
The purpose of this form is to collect information about the Small Business Applicant ("Applicant") and its principles. This form is to be completed by the Applicant and all individuals identified below and submitted to SBA LYNX PAYMENT.
SBA Lynx Payment is working to ensure fair and equitable access to its programs and will soon provide additional reporting on how the Agency is working to meet these goals. All the information is provided voluntarily by Business Owners to SBA Lynx Payment.
